HomeCare Instead proudly accepts the Community HealthChoices (CHC) Waiver, a Pennsylvania Medicaid managed care program designed to support older adults and individuals with physical disabilities who require long-term services and supports.

The CHC program helps eligible individuals receive the care they need while continuing to live safely and independently in their homes and communities, rather than in nursing facilities.

Who Qualifies for Community HealthChoices?

Individuals may qualify for the CHC Waiver if they:

* Are 21 years of age or older

* Are eligible for Pennsylvania Medicaid

* Require a nursing facility level of care

* Live in Pennsylvania

* Need assistance with daily living activities
